CHECK YOUR MAILBOXES FOR SENIOR T-SHIRTS & CARDS FROM LEADERSHIP!!
The leadership class wants seniors to write on a note card about their favorite memory at Liberty High School. They can also choose to decorate them if they please. The note cards will be found in the mailroom on Tuesday.
Turn in the note cards to Mrs. King's mailbox or RM 100.
All appropriate cards will be place on a huge poster and hung up for everyone to see!

Final
Transcript Request Instructions
A. Announce to Advisory:
Today we will be filling out a required document to inform the
counseling office what you plan to do after graduation and where to send your
final transcript.
A final transcript is a record
of your entire high school career. This
document will have your final GPA and Class Rank. Your college admission process will not be
complete without your final transcript. NCAA and NAIA also require a final
transcript.
Transcripts will be mailed
approximately 2 weeks after grades are due.
B. Process
Students
will log into their Family Connection
page.
a. Access
the FC site by going to the Counseling website
b. Click
on the About Me tab
c. Click
on the LHS Grad Survey on the left
side.
C. Explanation
of questions
1. Full
Name (Required)
2. Plans
for next year: State requirement (Required)
3. Area of
study: State requirement (Required)
4. College
application update and final transcript location
a. If a
student has used FC to request transcripts this year, their colleges (and
NCAA/NAIA) will be on their survey. The
student needs to update each college under the Final Decision column by indicating if they were Accepted or Denied. If they did not complete
the application process, they would check the box under Never Applied.
b. Students
will check the box under Attending of the college they
plan to attend in the fall. If the student is undecided, they should mark the
box of the school they will most likely attend.
(multiple checks are not allowed in this section)
c. Students
will check the college they would like their final transcript to be sent under Send
Final Transcript. College
athletes will also check the NAIA or NCAA box.
Undecided students will check each college they are still
considering. (multiple checks are
allowed in this section)
d. If the
student does not have their final college listed on their survey, the student
will click lookup in the next section
to add the college and then click the final transcript box.
5. Verification
of final transcript: Student will type in the name(s) of the college(s) they
have indicated in number 4. College
athletes will also include NCAA or NAIA.
Students will type NONE if they do not need their transcript sent to a
college. (Required)
6. Scholarships: Students will indicate amount and name, if
applicable. List all scholarships
including from colleges not attending.
